Uncle Junior, senile shots Tony during Season Six. Tony becomes comatose, dreams of being a salesman, and exchanges his suit case and becomes Kevin Finnerty. The story continues as Tony recovers, and changes and decides to mend ways. However, he faced lots of problems, when Vito Spatafore is considered outed because of being an homesexual, and later on, he had to deal with Phil Leotardo, now the new underground boss, because Johnny Sack is imprisoned. Tony failed to act, and Phil kills Spatafore. Tony’s family of crime is again on the verge of a bloody war.

Tony decides killing some of his associates because of minor troubles. Christopher wants to leave the mob, but keeps on relapsing into his drug addiction, compelling him to kill his sole friend from Narcotics Anonymous. Later on, he is seriously injured with Tony as the only passenger who suffocates him to death.
AJ’s fiancée leaves him, getting him into depression and had a failed suicide. Dr. Melfi was convinced by her friends, making use of the therapy as a sociopath benefit, as Tony does not make any progress, and stops seeing him as a patient.
